Onboarding Checklist (CNA • LPN • RN)

This checklist helps you get ready for contract placement faster. If you already applied, you can start gathering items now.

Required (most contracts)

  • Government-issued ID
  • Current license/certification (CNA, LPN, or RN)
  • Resume (recommended)
  • Social Security card or acceptable I-9 documents
  • Direct deposit info (voided check or routing/account)

Clinical & Compliance

  • BLS/CPR (if required by facility)
  • TB test (within facility-required timeframe)
  • Immunization records (facility dependent)
  • Background check authorization
  • Drug screen (as required)

Nice to have

  • Professional references
  • Skills checklist / competencies
  • Prior employment verification
  • Any specialty certifications
  • Work availability calendar

W-2 vs 1099 (quick note)

Some contracts are W-2, some may allow 1099 depending on the assignment and facility rules. Tell us your preference in the application — we’ll confirm what’s available for each contract.
